Health Resource Center
Brooklyn Center's Health Resource Centers offer free and low-cost medical, dental, and mental health services to the youth of Brooklyn Center schools and community ages 0-19.
To receive services, a completed written and signed consent form is needed by the parent or guardian and is required annually for all children and youth for the following services: medical treatment, including immunizations, dental treatments, eye-exams, and mental health services. Consent forms are available below, in school health offices, and at the Health Resource Centers. Please be aware that Minnesota Law does allow young people to receive treatment, without parent/guardian consent, except for sexual health and chemical dependency concerns.
MEDICAL SERVICES
Park Nicollet provides free walk-in medical services on Tuesdays and Thursdays from 1-5 p.m. (no appointment needed) during the school year. During school breaks, walk-in medical services may vary. We are closed on major holidays and during emergency school closures, such as snow days. The Annex Teen Clinic is available twice per month for appointment-based sexual health services. A teen health mentor is on site every day for questions.
DENTAL SERVICES
Children’s Dental Services provides a full range of restorative and preventative care and are on-site once to twice a month Provides care for the insured and will work with those who are uninsured on a sliding scale discount program Services are available during the school day for BCCS students as well as for the community (0-26 years old or pregnant). EYE CARE
A Chance to Grow provides a full range of vision services and are on-site twice per month Provides free or low cost care for the insured and uninsured patient Call 763-450-3385 ext. 5100 for more information.
MENTAL HEALTH
Mental Health services are provided by The Family Partnership, Arubah Emotional Health, North Psychology Clinic, Cornerstone MN, and POR - Emotional Wellness and independent providers. Appointments are available for free or low cost for insured and uninsured patients at Brooklyn Center Secondary, Brooklyn Center Elementary School, and the Early College Academy. Entrance Information: When entering through Door #16, ring the doorbell on the callbox attached to the building. You will speak with a staff member and they will come to the entrance doors to let you in and walk you to the clinic office.
Additional Information: Students must be accompanied by a parent/guardian during their appointment.
